HIV Medication Keeps Everyone Safe
Updated: May 25, 2026. Full Article.
Welcome to the ultimate black hole of global travel. The Kim regime completely denies that gay men even exist within its hermetically sealed borders, scoffing at same-sex attraction as nothing more than a "decadent Western mental illness." If there is a scene here, it is not behind closed doors - it is locked behind the doors of perception. There is no talk of it, and when men inevitably 'know' each other, they literally know not what they do. Dating apps are of course stone-dead behind the state firewall, and a total information blockade keeps local men entirely ignorant of the concept of gay sexual desires. Imagine that.
We only know of this from the legendary testimony of the country's only openly gay defector, Jang Yeong-jin, who revealed that citizens routinely live half their lives without even knowing the word "homosexual" exists. Even so, it is impossible to believe that the absence of words would ever stop anyone from having same-sex attraction, whatever they want to call it. I mean, there must be some good-looking guys there. In what can only be called a state of denial, Jang Yeong-jin also says, "In the DPRK men spend a long time in the military, physical contact is frequent between the same sex and they experience special emotional bonds. This isnβt to say that all these men are gay, but rather, they are tied by a special comradeship". Let that hang for a minute. For any traveler stepping inside, expect a sterile experience and a social lockdown where keeping things low-key isn't just polite - it's mandatory. Sounds like fun!

Urgent (PEP): If you think you have been exposed to HIV, you must start PEP within 72 hours.
Welcome to the ultimate fiction factory of global healthcare. On paper, the Kim regime proudly brags to the world that North Korea has exactly zero cases of HIV thanks to its "superior socialist system." And of course, it is superior. I've never heard of a dictator saying his country is not the best ever - nobody has seen anything like it... The harsh reality on the ground is not so funny. High-authority investigations from groups like Amnesty International paint a dire picture of a medical infrastructure in total systemic collapse, with hospitals operating entirely without electricity, heat, or basic sterile supplies. For ordinary citizens, specialized sexual health clinics or anonymous testing don't exist. They've probably never heard of such a thing. If you are a local, disclosing any same-sex activity or seeking an STI test from a state doctor doesn't just breach confidentiality codes - the ethics go out the window, as the laboratory - if it gets that far - will inform the state and that triggers an immediate ticket to a psychiatric isolation ward or hard labor for being corrupted by "Western cultural decadence" - Both avenues leading to a probable death sentence.
If you are living in a country where it is impossible to get care and you're panicking about HIV right now, stop and look at this screen. This screen is here for you. There is a lot of outdated opinion about HIV out there, but it is just homophobic rubbish. Take a slow, deep breath. You have time. You are going to be okay and we are with you. HIV is a completely manageable medical condition. With modern medication, you will live a long, healthy, and happy life. But you must avoid stress, it is your absolute worst enemy right nowβit will make you weak. I know that's easy to say, but just try following the breathing exercises below. They really work. Protect yourself: rest, eat a meal, listen to music, and do whatever you must to quiet your mind. Force a smile, and within a minute you will start to feel better. I know, but it works. Do it.
If you are trapped in a hostile country and local state doctors are not safe for you, your priority is to move yourself to a safe environment. I know that's not easy, you might think it's impossible, but you are not alone. International organizations exist solely to help gay men like you to escape dangerous regimes. They can really help you and you can get your life-saving healthcare. Take a deep breath. You have time but you must take action as soon as possible.
The Real Deal: Here is the raw truth the regime tried to bury. Independent tracking reported by Global Citizen exposed that North Korea is secretly wrestling with an active epidemic of at least 8,400 HIV-positive individuals, with case numbers climbing relentlessly. Because the state lacks basic diagnostic equipment and safe blood banking protocols, the virus is heavily driven by reused medical needles and contaminated state blood donations, alongside underground survival sex work and illicit drug use. If a local citizen gets flagged as positive, the state response is swift and draconian. Paranoid authorities, completely terrified of infectious outbreaks, subject infected individuals to strict involuntary quarantine, forced detention, or immediate social exile. To make matters grimmer, international sanctions mean up to 40% of smuggled antiretroviral drugs get seized at the Chinese border, starving real people of life-saving treatment. Traveling visitors must pack a personal medical supply kit, as seeking local help for HIV care will instantly trigger a state security nightmare. A nightmare for you too. Better still - What are you thinking of? Don't go!

If you are looking to paint the town red in Pyongyang, pack a massive bucket of gray paint instead. There is absolutely zero nightlife, no hidden back-alley bars, and a total vacuum where a gay community should be. The local gay scene in Korea, as it was known back in the day, was actually at the palace. Korea, like everyone else, has had a few gay kings, the most famous example being King Gongmin (r. 1351β1374). His palace was called Manwoldae (which translates to the "Full Moon Platform"), and honestly, I'm not making any of this up. After the death of his beloved Queen Noguk, King Gongmin reportedly lost interest in women and established a group of handsome male bodyguards known as the Jajewi. He is recorded as having 'engaged' in sexual relationships with these young men. Well, yes. Gay rulers have been known to do that. This history is so well-known that it was the subject of the 2008 movie A Frozen Flower. I'm guessing it was cold at night. But we're getting off track.
Gay men - who are not gay - can technically book a trip through tightly monitored, state-approved operators, but make no mistake: you are trapped inside the theatre of Kim's making. You will be assigned two government minders who will stay glued to your side from dawn until dusk. Could be worse. Location-based dating apps like Grindr or Tinder are just not available, so forget about them during your stay because you won't have access to the open internet. But you can cruise, right? Well, attempting to cruise local men - or make eyes at your security guards - is a direct, one-way ticket to a grueling hard-labor camp - don't misunderstand the word camp. Best to sanitize your phone of all those great gay pics before you enter the Hermit Kingdom.
The first thing you might notice during your trip to North Korea after a day of absolute fun on your sight-seeing trip is the amazing quality and entertainment offered by the national TV channels. There are currently four channels that keep you amused with subjects ranging from state documentaries, educational programs, educational programming (no, I didn't repeat that twice, those are the second and third channels), and sport, of course. A positive delight for those who like to fall asleep with the telly on.

Your vacation here is strictly a tour of the bizarre. Stick to the state-sanctioned historical itineraries, marvel at the gargantuan concrete monuments, and nurse a lonely pint of cheap Taedonggang beer at your designated hotel bar while your minder tracks your every gulp. Enjoy the nightlife, because that bar you're sitting in is it.
Total state surveillance is active every single second. Hotel rooms are wired, and border guards love watching you get dressed - bring some pajamas. An absolute no-no: Do not bring in any foreign media, unvetted digital files, or physical publications containing anything that could be considered explicit material. Smuggling adult content or pornography is viewed as an aggressive act of subversion, and it is a major state crime that can carry a literal death sentence.
Really? Oh, yes. There have been some high-profile 'detentions' but not since Otto Warmbier was seized in 2016 - he tragically died in 2017 shortly after being returned to the States as a direct consequence of his time in one of Kim's prisons. However, we could not find data on 'low-profile' arrests of tourists. Our doomy conclusion is that this doesn't mean there haven't been any.
Let's be honest, this country is a horror show. It's so bad that it is even hard to empathize because that requires some ability to imagine the terror that our community must be suffering there. It is literally unimaginable. No, you should not consider going - to step foot in the place is paying homage to the Kim - and that just cannot be. However, if you do go, don't forget to drop me a line and tell me how it went!
The paranoia of the regime extends heavily to non-conforming gender expressions and same-sex relations among women, which are aggressively targeted as toxic examples of capitalist contamination. According to historical field reports from The Korea Times, the state has gone so far as to publicly execute lesbian women under the guise of purging foreign ideological pollution from the population.

Street safety for gay men is dictated by absolute cultural erasure with zero official gay-specific arrests reported per year due to complete state denial. Any visible non-conforming behavior is aggressively crushed under sweeping anti-socialist laws that mandate severe state punishments.
